COVID Care Protocols
If you're experiencing COVID-like symptoms, please call 405-734-2778 before coming into the Medical Group.
Based on symptoms, you may be referred to the Respiratory or Pediatric Sick Clinic for evaluation.
Services Provided
We provide immunization services to all TRICARE beneficiaries. Patients should bring in all immunization documentation when visiting the clinic for the first time or if services were received from a non-Department of Defense facility.
Walk-in Policy
All services are provided on a walk-in basis, and patients may experience increased wait times depending on patient volume.
COVID-19 Vaccines are also offered on a walk-in basis.
- Pfizer (ages 12+) offered Monday—Friday
- Pfizer (pediatric) offered Wednesdays only
- Moderna (ages 18+) offered Fridays only
Obtaining Records
The best way to obtain a hard copy shot record is to come into the clinic during normal business hours and pick one up, this way it can be stamped and signed making it official. For anyone in your family over the age of 18, they will need to be present when requesting their shot record.
Fax a Record
To fax a shot record to an off base facility we require a Medical Information Release Form (HIPAA Release Form) to be filled out and signed by the requester and then provided to our clinic.
We will only fax to a school, primary care manager, or health clinic. Without a signed release form we are prohibited from discussing any aspect of your medical information.
Records Access for Active Duty Members
Active duty members, you can access you and your family members by clicking on My IMR / ASIMS - Medical Readiness & Deployment Health.
Note: This site is restricted and requires a common access card. Users without a common access card will receive a website error message.
Vaccine to Medical Records
To have vaccine records transcribed into medical records, please email our clinic’s workflow at usaf.tinker.72-mdg.mbx.immunizations@mail.mil.
